Boston Community Health Needs Assessment Survey

The Boston Community Health Collaborative is gathering input from Boston residents to understand the health strengths, challenges, and priorities of our communities. Take our survey today!

About the Survey:

The Boston Community Health Needs Assessment is a comprehensive effort to identify the strengths, challenges, and public health priorities for residents of Boston. This process is led by the Boston Community Health Collaborative, a multi-sector collaboration of Boston nonprofit teaching hospitals, community health centers, community organizations, and public health. Boston Public Health Commission is the central coordinating agency for the Boston Community Health Collaborative.

The community health survey asks residents about their community health priorities, access to care, and availability of resources to support their physical and mental health and wellbeing. By taking this survey, you can help shape health plans and funding priorities to support your community's health. The information gathered from the survey helps our local public health system in guiding the work towards making Boston a healthier city for all.

This year, the community health survey will include a raffle for prizes, including two $100 grocery gift cards, museum passes, family-friendly experiences and more. Thank you for your partnership and support!

Who is eligible to take the survey? 

  • Anyone aged 14 or over who lives in Boston is invited to take the survey.
  • Participants can take the survey using a computer, phone, or tablet. Paper surveys will also be available at a number of community events this Fall.
  • The survey is live and currently available in English and 8 additional languages: Arabic, Cape Verdean Creole, Haitian Creole, Portuguese, Spanish, Somali, Simplified Chinese, and Vietnamese. 

How long will the survey be available?

The survey closes early December 2024. 

When can we expect results?

The community survey is one component of the Community Health Needs Assessment process, which help to understand the community’s health experiences. Results of the survey and the Community Health Needs Assessment Report will be available during Summer 2025.
